    This can happen due to a SharePoint issue when apps are uninstalled and reinstalled. The cache for the ribbon commands is not refreshed. It does not occur though when just updating an app.

     

    To resolve this you can invalidate the browser cache Clear Browser Cache.

     

     

    Alternatively you can initialize customizing the forms using the main app page (select the Ardevia Rich Forms app from the Site Contents page).
