SubLists - Not seeing lookup column as list sublist

Hi

 

I am trying to add an editable grid to my form.

 

The Grid should contain the following columns:
Medication name, Dose, Route, Frequency, Type

 

I have create a list with these each of these colums called Medication Details

 

From my main List (or Form)  I have a look up column that references the Medication Details list.

 

When I go to insert sublist I am told that my list "does not have any related lists"

 

I would be grateful to see where I might be goin wrong.

  The lookup should be on the Medication Details list referencing your main list.

  The Sublist should capture details of 1 or more medications involved in this particular adverse incident.
  and shoud be unique for each Form.

   

  When I go to View or Edit List INITIALLY I can see the correct medications for that particular List.

   

  Problem: When I his the Browser Page Back Arro (upper L screen)  all sublist items populate the sublist on the form.

   

  Please advise,
