Page does not load correctly - if the site collection isnt on port 80

if i install Rich Forms on a site collections like http://sharepoint/site/test everything works fine.
if i install it on http://sharepoint:12345/sites/test
and i click on Rich form it does not load correctly

  This is due to the absence of a root site: when there are no such site, some scripts cannot be loaded.
  Please add the site with an url "http://sharepoint:12345" . This should fix the problem
