Insert Description lines: Creation by, last Modification by

Hello Everyone!

 

Anyone know how to insert or make visible the last lines of a record ?, lists without Ardevia shown last lines creation date and last modified but once modified form are no longer visible these lines.
For Example

 

Type of content: Event
Version: 2.0 
Create :28/01/2016 19:09 by USER_EXAMPLE
Last Modification: 28/01/2016 19:09 by: USER_EXAMPLE[/i]

  This lines is a part of toolbar control. You can insert it from ribbon (Rich Forms tab -> Insert section -> Toolbar)
