    Hello Rick,

     

    It looks like something went wrong during App upgrade.
    I recommend you to remove Rich Forms from your SharePoint site and add it again. All configuration will be preserved.

    I removed the app, then re-added it, but still getting the "page can't be displayed" when I click on it. I upgraded my browser to IE11, too. Windows 7 sp1 is the operating system.

    This issue can be occurred in SharePoint on-premises environment when apps isolated domain is not configured correctly. Please look at the following topic how to configure environment for SharePoint apps:
    Configure an environment for apps for SharePoint (SharePoint 2013)

     

    Please install any other [u]SharePoint-hosted App[/u] from MS Store and check whether it will work. For example, you can add any free trial skybow app, this vendor provides SharePoint hosted apps:
