    Hi Williie,

Could you please provide us more details about this issue:
    1) Please send us screenshot
    2) Is there any jscript error on page?
    3) Check the behavior in other browser like Mozilla, Ghrome, etc

    Please send us the result.

As Ardevia Rich Forms is not anymore available in the MS Office Store, I would recommend you to upgrade Ardevia Rich Forms to skybow Rich Forms. Please read the following blog for more details:

I've tried different browser but still got the same result and I don't see any script issues.

    Missing Textarea

I also tried the suggestions here

Version 2.8.0.0 Execute script action is not working

The textarea come up but it breaks everything. For example, when I click a list item for display, it just kept saying working on it forever. So I eventually roll back to the original version.
